5/10/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted JaCaeri upon arrival to the home setting. Upon arrival to the home setting, JaCaeri appeared to be in a content mood evident by his facial expressions such as a smile on the face. QMHP-C engaged in him a discussion to determine if he has been complying with his medication management. He verbalized to QMHP-C that he has been taking his medication on time. Mrs. Shabazz reports that he continues to struggle with controlling his anger when he is asked to clean his room or to assist within the home with household chores. She reports that he left the home leaving her to search for him in the neighborhood. 2. Behavior: JaCaeri presented cooperative behaviors as he did not need any redirection from QMHP-C as well as him answering open -ended questions and being praised by QMHP-C. At times he became withdrawn from the session due to him getting upset with his mother when reporting his behaviors. 3. Behavior: During this portion of the session, JaCaeri struggled to ignore distractions and finds himself off task and interrupting others who are trying to learn. 1.Intervention: QMHP-C processed with JaCaeri his current views towards authority figures and reviewed with JaCaeri the difference between authority figures and adults. QMHP-C explored with JaCaeri the difference in respecting the rules of someone and agreeing with what they believe. QMHP-C encouraged JaCaeri to be unique and creative, while recognizing that everyone has to follow rules. 2. Intervention: QMHP-C explored with JaCaeri the different roles he holds within his family and what those roles mean for everyone. QMHP-C encouraged JaCaeri to refrain from talking back and to think of alternative things to do with his mouth and voice when he feels the urge to say something negative being as though he is young and still has a lot of growing and understanding. 3. Intervention: QMHP-C used positive regard as he told JaCaeri that he appeared to have positive home management in class earlier in the day. QMHP-C informed JaCaeri of more acceptable and expected responses to situations that would normally lead JaCaeri to be triggered and display inappropriate, negative behaviors while in the home. QMHP-C assisted JaCaeri to behave in more positive ways by addressing the conflict he was facing with his mom. QMHP-C informed JaCaeri of replacement behaviors in efforts to decrease attention seeking behaviors JaCaeri often is involved in. QMHP-C modeled appropriate ways for JaCaeri to get positive attention from his family. QMHP-C inquired of JaCaeris’ current mood. QMHP-C utilized active listening skills as JaCaeri spoke of his current mood. QMHP-C will educate JaCaeri on behavior modification to help JaCaeri identify what behavior he needs to decrease and avoid daily. QMHP-C will utilize behavior modification system to aid JaCaeri with following directions given by authority figures. QMHP-C will meet with JaCaeri daily to discuss progress and expectations that are working and not working in the home weekly. 1.Response: JaCaeri appeared to be receptive to QMHP-C’s interventions and services that were provided to him during the session. While exploring JaCaeri’s feelings, JaCaeri mentioned to QMHP-C that he was in a good mood. JaCaeri stated that he knew the difference between authority figures and adults. 2.Response: JaCaeri discussed with QMHP-C that he talks back to his mother because she is always yelling at him and his siblings. When asked what he likes to do in his spare time, he discussed with QMHP-C that he likes to play his video game when he feels the need to say something back to his mom at which she will take that from him at times. 3. Response: JaCaeri expressed appreciation to QMHP-C by smiling and giving good eye contact. JaCaeri expressed that he should receive an incentive for his display of desirable behaviors throughout the day. JaCaeri did not engage in any disruptive activities or behaviors. JaCaeri was able to maintain his composure when he was confronted with the potential for conflict. JaCaeri was observed engaging in several conflict resolution skills by walking away and telling an adult first. JaCaeri verbalized that he’s able to focus better when it is quieter and more peaceful. JaCaeri took his mind from thinking and doing to a place of concentration, which allowed him to focus on one thing at a time instead of being all over the place and getting easily distracted. JaCaeri demonstrated more acceptable responses, avoiding anger provoking situations in the home and community.
